Step 2: Obtain Referral: All Physical therapists in the state of Texas are required to have an Prescription signed by a provider with prescriptive rights within 10-14 business days depending if provider is residency trained of seeing the physical therapist.

Step 3: Required Forms: You will receive an email after being scheduled verifying your appointment day and time along with a link to our portal to fill out consent forms. If you have trouble filling out the forms, please feel free to contact our office, or print off the forms below and bring them filled out with you.
Step 4: At the clinic: Arrive at your session early for patient check-in. This can take 5-10 mins as long as consent forms are filled out ahead of your appointment time via online or filled out at home.

We are an out of network practice for all insurances other than Medicare and Tricare. We are happy to submit claims to your insurance provider on your behalf, but payment is due when services are rendered.
Many have the impression that we only treat women because they are the only ones with “pelvic problems.” Well, this is a sad misconception. We not only treat women during pregnancy, post-partum, and later in life for a variety of reasons, but we also treat men that are experiencing pelvic pain, incontinence, and/or constipation. We also treat kids that are experiencing constipation, pain or urinary symptoms.

On your first visit we will collect quite a bit of information from you, so please bring any prior tests or diagnostics run such as colonoscopy’s, ultrasounds, MRI’s, Xrays, etc because we will review them and ask you questions. Then we will perform an examination within your comfort level. With the evaluation there will be some movement and testing involved, wearing comfortable clothing would be helpful.
If you need to cancel or reschedule an appointment you must do so within 1 business day of the scheduled appointment time or we will charge a $50 fee to help us cover the cost for that session to the card we have on file. If you NO Show a visit or cancel within 2 hours of your scheduled appointment time, we reserve the right to charge you the entire cost of the visit to the card we have on file.
